PROFESSIONAL QUALIFICATIONS / RESPONSIBILITIES. All Contractor HCP's shall meet the qualifications for that specialty and perform essentially the same functions, within the scope of acceptable for their technical professional discipline nd standard, as those required by Army or government service technical professionals of similar experience and in similar duty
assignments. Contractor HCP practices/productivity will be compared to that of other Contractor HCP assigned to the same medical center.
 The Clinical Pharmacist shall possess and maintain a current, active, valid, unrestricted license for the practice of
pharmacy in at least one state or territory in the United States.
The Clinical Pharmacist shall hold a post-baccalaureate or entry level Doctor of Pharmacy (PharmD) degree
from an accredited college of pharmacy and at least one of the following:
 Post-Graduate Year 1 (PGYl) Pharmacy Practice Residency from a program accredited by the
American Society of Health Systems Pharmacist (ASHP).
Board certification in one or more specialties recognized by the Board of Pharmacy Specialties (BPS).
(3) Minimum of3 years of post-graduate experience as a clinical pharmacist with at least 50% oftime spent in direct patient care related clinical activities such as medication therapy management, coordination of care, patient advocacy, wellness and health promotion, triage and referral, and patient education and self-- management services.
